Health Readiness Check

A few yes/no questions to make sure it’s safe for you to start, and where to start. This is not a medical exam.

About you

Are you 18 or older?

Under 18? A parent or legal guardian must read and sign this form too (see the bottom).

Step 1 · The official PAR-Q+

The PAR-Q+ is the international standard “ready to exercise?” form. It’s free and takes about 5 minutes. We link to the official version because it may not be changed or copied.

Open the official PAR-Q+ (eparmedx.com) ↗

My PAR-Q+ answers were:

If you answered YES to any PAR-Q+ question, talk with your doctor by phone or in person BEFORE you start becoming more physically active or BEFORE you start working with a Personal Trainer. Please email or bring the completed PAR-Q+ to your first session.

Step 2 · A few more questions

In the last 3 months, have you exercised on purpose at least 3 days a week for 30 minutes or more?
Has a doctor ever told you that you have heart disease, diabetes (type 1 or 2), or kidney disease?
Do you ever have any of these, at rest or when active? (check any)
Has a doctor cleared you to exercise?

If you checked a symptom, or said YES to heart, diabetes or kidney disease, please get your doctor’s written OK before training. If a symptom happens during exercise, stop and tell your coach right away. Call 911 for chest pain that doesn’t go away.

Done? Send it to Chad

“Send to coach” opens the email app on your phone or computer (like Mail or Gmail) with your answers already typed in. Look it over, then press Send in your email app. Nothing is sent until you do, and this page doesn’t save your answers anywhere.

Email app didn’t open? Tap “Copy my answers”, then paste them into an email to triumphanttransformation@gmail.com. Or save as a PDF and attach it.

Not medical advice. Talk with your doctor before starting any exercise program, especially if anything on the Health Readiness Check applies to you. “I can do all this through him who gives me strength.” Philippians 4:13 (NIV)
